Program Description
As a Pediatric Primary Care Learning Collaborative participant, you will have access to a series of web-based learning collaboratives and an optional Quality Improvement (QI) project. Upon completion of each learning collaborative requirement, you will be eligible to receive MOC Part 2 and CME credits. Upon completion of the optional QI project, you will be eligible to receive MOC Part 4 credits.
Learning Objectives

Apply strategies to screen, identify, diagnose, treat, and manage common conditions seen in pediatric primary care, incorporating updated evidence-based guidelines and interprofessional collaboration.

Recognize and utilize clinical pathways for effective management of common pediatric conditions, ensuring alignment with team-based care practices.

Identify and apply appropriate referral guidelines for specific conditions to support seamless coordination between primary and specialty care providers.

*Participants will also have the option to implement a quality improvement project focused on using the Survey of Well-Being of Young Children (SWYC) ages 0-5, as the intervention tool. The Care Network Quality Improvement team will work with each participating practice to facilitate quality improvement activities, collect and analyze data, address any questions and provide ongoing support. Participants will commit to 5 QI visits facilitated by the Care Network QI team to support implementation and improvement efforts.

Participants will take part in a series of web-based, educational sessions led by faulty from the Neurological Institute at CHLA.
LEARNING OBJECTIVES
Expand your capacity to identify, treat and manage common neurological conditions in primary care pediatrics. Use clinical pathways to manage common neurological conditions within primary care, as well as implement referral guidelines for specific conditions.
